1) Letter to parents/guardians regarding the visit
2) Letter on National Childhood Immunisation Schedule 2008
3) Information booklet on “School Health Service: Health Check and
Immunisation” for P5 pupils
i) Consent form for immunisation (found inside the Information
Booklet, pages 1 & 2) (please see pictures below )

Please sign on the consent form and remind your child to return it to me tomorrow.
